Hi Rin!
No doubt someone from PATHS will weigh in on this, but from what I understand, the 'name most lifeforms know you by' is THE name you should use when signing up for PATHS. 
This has Everything to do with the receptivity your subconscious will have to the download of instructions. I think of it as a potential person, a stranger even, knocking on your door and asking to be let in. If you don't recognize them, you won't likely allow them into your home.
In this case, the 'person' is the millions of PATHS instructions, and your 'home' is your subconscious - if the subcon doesn't accept that you are the intended recipient, the door stays shut - so make sure that the name most lifeforms know you by, the location you believe you were born in, and the date you believe you were born on, are Correct! I say 'believe you were' because its only what your subcon believes that really matters. In some ways, I liken the subcon to an obedient dog  that believes and listens to what you tell it, almost instinctually, rather than intelligently. In other cases, the collective subconscious appears to show great intelligence, intent and purpose to actively assist us - it Wants to and will deliberately make effort to do what you ask it.
We spend our lives telling our subcon thousands of things that are not intrinsically 'true'. Things like we are stupid, fat, ugly, slow, unloved, limited - yet none of these things are 'true' unless we believe it. The beautiful thing is that we can just as easily begin to tell ourselves the opposite of those things, with NO limits, and those things the subcon will, sooner or later, begin to accept and believe and 'miracles' can begin to happen!!! 
To me, the current mind-blowing pinnacle of current knowledge on how untrue all the things that institutions (including classical science), parental figures, authority-types have told us, is in technologies like PATHS and the modality of Matrix Energetics (ME). With these current ideas, we can learn that nothing is as it seems in the physical 'reality' and that there are No limits except the ones we believe! Yes, what you believe, your conscious mind creates as your reality. There is no unquestionable reality that exists as incontrovertible 'truth' in this physical world. The PATHS technology opens us up to believing this as well as actually causing changes in us physically and mentally to acting on it. ME can show how to alter one's perception of 'reality' to dip into the quantum field to create through intention anything that one wants! Amazing and wonderful stuff 
What we pay attention to, becomes our reality and this is why affirmations have a limited and slower-actualization to personal change. A new affirmation, especially if it is radically different than what we currently believe is who we are and part of our reality, is actually a statement to the universe that you believe you are not the thing that you are affirming, but wish it to be. Perhaps something that will make affirmations to work better is to do the one extra step of building a foundation of 'knowingness' that you have no limits and that all things are possible, if you only truly have one thing to do, and that is to Allow them to be. From that start, I believe affirmations are much more effective.
